BOTH-LEG SWELLING WITH MULTIPLE POSSIBLE CAUSES
Swelling of Legs (Bilateral)
Swelling of both legs can indicate underlying heart, kidney, liver or venous conditions, or be due to medications, prolonged standing or hormonal changes. Workup guides targeted treatment.
ABOUT THIS CONDITION
What is Bilateral Leg Swelling
Bilateral leg swelling (oedema affecting both legs) is a common medical complaint with a wide range of possible causes. Common causes include heart failure (right-sided or biventricular), chronic venous insufficiency, kidney disease (particularly nephrotic syndrome and chronic kidney disease), liver disease (cirrhosis with low albumin), thyroid disorders (particularly hypothyroidism), medications (especially certain blood pressure medications and anti-inflammatory drugs), prolonged standing or immobility, obesity, pregnancy and lymphatic problems. Less commonly, it can be due to severe sleep apnea, severe vitamin or nutritional deficiencies, or hormonal conditions. Typical features include progressive swelling of both legs, sometimes extending to thighs and abdomen, often worse in the evening or after prolonged standing, possible pitting (indentation when pressed) and associated features of the underlying cause such as breathlessness in heart failure, frothy urine in nephrotic syndrome, or jaundice in liver disease. Diagnosis involves detailed history, examination including assessment of heart, lungs, liver and kidneys, and targeted blood tests, urine tests and imaging. Treatment is highly cause-specific. Dr. Patnam Pravallika Reddy provides comprehensive evaluation and care at Lux Hospitals, Hyderabad.

HOW WE TREAT IT
Treatment Approach
Systematic Workup with Cause-Specific Treatment
The most effective approach is a systematic workup including detailed history, examination and targeted blood tests, urine tests and imaging to identify the underlying cause, followed by cause-specific treatment combined with general measures such as leg elevation and compression where appropriate.
- 1
Consultation & Assessment
Dr. Pravallika reviews symptoms and possible causes, examines the patient and arranges blood tests, urine tests and imaging including chest X-ray, ECG and echocardiogram when needed.
- 2
Treatment Planning
A personalised plan is created based on the identified or likely cause, with specialist referral when needed.
- 3
Medical Management
Cause-specific therapy, fluid and salt management, diuretic therapy when appropriate, compression and elevation measures.
- 4
Recovery & Follow-up
Regular review of symptoms, weight, swelling and treatment response, with ongoing management of underlying conditions.

Treatment Options
Identifying the Underlying Cause
Detailed history, examination and targeted investigations — blood tests for kidney, liver, thyroid and cardiac function, urine tests, chest X-ray, ECG and echocardiogram when needed — identify the cause.
Cause-Specific Therapy
Treatment is directed at the underlying cause — heart failure management, kidney or liver disease treatment, thyroid replacement, treatment of venous insufficiency or other.
Diuretic Therapy When Appropriate
Diuretic therapy may be used in selected cases such as heart failure or significant fluid overload, under careful medical supervision.
Compression and Elevation
Compression stockings (when not contraindicated) and elevation of legs significantly help in venous insufficiency and dependent oedema.
Lifestyle Modification
Reducing salt intake, weight management, avoiding prolonged standing and regular gentle activity all support better fluid balance.
