What is Hypocalcemia

Hypocalcemia refers to a lower than normal level of calcium in the blood. Calcium is essential for normal nerve and muscle function (including the heart), bone health, blood clotting and many cellular processes. Common causes include vitamin D deficiency, parathyroid gland disorders (hypoparathyroidism, often after thyroid or parathyroid surgery), chronic kidney disease, severe magnesium deficiency, certain medications, pancreatitis, severe sepsis and massive blood transfusion. Typical features depend on severity and rate of onset and include tingling and numbness around the mouth, fingers and toes, muscle cramps and spasms (particularly in the hands — known as carpal spasm), muscle twitching, tiredness, anxiety, and in severe cases, seizures, abnormal heart rhythms and altered mental state. Mild chronic hypocalcemia may cause no symptoms. Diagnosis is confirmed with blood tests for calcium (corrected for albumin or ionised calcium), along with tests for vitamin D, parathyroid hormone, magnesium and other causes. Treatment involves calcium supplementation (oral or intravenous depending on severity), vitamin D supplementation, treatment of the underlying cause and correction of magnesium deficiency if present.
